Gestão do erro terapêutico na pessoa em situação crítica: o contributo do enfermeiro especialista

Abstract

O presente relatório de estágio, desenvolvido no Mestrado em Enfermagem em Associação, na Área de Especialização em Enfermagem Médico-Cirúrgica, vertente Pessoa em Situação Crítica, evidencia a aquisição e consolidação das competências do Enfermeiro Especialista e de Mestre em Enfermagem. O erro terapêutico constitui causa de eventos adversos em cuidados intensivos, representando um desafio para a segurança do doente. Sustentado na Teoria da Sabedoria Clínica em Cuidados Agudos e Críticos, de Patricia Benner, o estágio decorreu no Grupo Local do Programa de Prevenção e Controlo de Infeções e Resistência aos Antimicrobianos e no Serviço de Medicina Intensiva. A metodologia assentou numa abordagem ativa e reflexiva, combinando scoping review, diagnóstico de necessidades formativas, formação em serviço, elaboração de norma regulamentar, auditoria e vigilância epidemiológica. Os resultados evidenciaram subnotificação de incidentes, cultura punitiva e fragilidades na segurança terapêutica, salientando, pela perícia clínica, o contributo do enfermeiro especialista na segurança e qualidade dos cuidados; Management of therapeutic error in the critically ill patient: the Contribution of the specialist nurse - Abstract: This placement report developed within the framework of the Master’s Degree in Nursing in Association, in the Specialisation Area of Medical-Surgical Nursing, focusing on the Person in Critical Situation, demonstrates the acquisition and consolidation of the competences of the Specialist Nurse and of the Master in Nursing. Therapeutic error constitutes a cause of adverse events in intensive care settings, representing a challenge to patient safety. Grounded in Patricia Benner’s Theory of Clinical Wisdom in Acute and Critical Care as its theoretical framework, the internship took place in the Local Group of the Programme for the Prevention and Control of Infections and Antimicrobial Resistance and in the Intensive Care Unit. The methodology was based on an active and reflective approach, combining a scoping review, training needs assessment, in-service training, development of a regulatory standard, audit, and epidemiological surveillance. The results highlighted underreporting of incidents, a punitive culture, and weaknesses in therapeutic safety, underscoring, through clinical expertise, the contribution of the specialist nurse to the safety and quality of care.
